You may want to try the latest 7.05 GPL Ghostscript release.
In general, input fields are not a problem. If you have a
PDF file that 7.05 cannot handle, please send it to us on
the bug-gs at ghostscript.com mailing list.
We've done quite a bit of work to make Ghostscript handle pretty
much any PDF file (including some 'broken' PDF documents that
violate the spec., but that Adobe Acrobat Reader can handle).
