Its possibly a bug, or possibly a corrupt/broken/invalid PDF file, without 
seeing the file its impossible to tell. Does Acrobat open the file ? Does 
any other PDF consumer (eg xpdf) open it ?

If you are reasonably sure that the file is valid, the best thing to do is 
raise it as a bug. If you go to :

http://bugs.ghostscript.com/enter_bug.cgi

You can raise a report. We will need to see the PDF file, after you open 
the report you can attach the file, if its sensitive then after you've 
attached it, mark it as private. That way only Artifex staff can open the file.
